Indiana BMV requires proof in four core categories for a learner permit: identity, Social Security number, lawful status and Indiana residency. Residency normally requires two unique documents.

Bring two unique residency proofs
If you cannot provide two qualifying documents, BMV describes an Indiana Residency Affidavit route with additional requirements for the person signing it.
Minor financial-liability signature may also apply
A minor's permit or license application requires the applicable financial-liability agreement and identification from the adult signing it.
