Documents for an Arizona instruction permit
Arizona requires proof of identity, age and authorized presence under federal law for driver-license credentials. Applicants also provide the Social Security number required by state law and the residency documents applicable to the credential.

Residency requirements depend on credential type
Arizona Travel ID has its own document checklist and requires two proofs of Arizona residency. Standard credential requirements should be checked against the current MVD identification form.
Minors need parent or guardian approval
After a teen passes the permit test, MVD requires the parent or guardian approval process described on the Permit Test page; if the approver cannot attend, Arizona provides for a notarized application.
